Add model card, link to paper and code

This PR adds a model card, by ensuring that the model is linked to the paper at https://huggingface.co/papers/2505.20298.
It also adds a pipeline_tag, enabling the model to be found at https://huggingface.co/models?pipeline_tag=image-text-to-text&sort=trending.
Lastly, it adds a link to the Github code repository.
